The most common cell pattern is given a grade of 1 (most like normal cells) to 5 (most abnormal). The sum of these two numbers is your Gleason score. Your Gleason score can range from 2 through 10. Most of the prostate cancer cases diagnosed today have Gleason grades of 5, 6, or 7. The more aggressive forms of prostate cancer have scores of 8, 9, or 10.

Your Gleason score isn't a separate test. It's a number based on the results of your biopsy. You usually get it when you're first diagnosed with prostate cancer. In most cases, scores range between 6–10. Prostate cancer grade (Gleason score or Grade Group) If prostate cancer is found on a biopsy, it will be assigned a grade. The grade of the cancer is based on how abnormal the cancer looks under the microscope. Higher grade cancers look more abnormal, and are more likely to grow and spread quickly. There are 2 main ways to describe the grade of High risk: Tumor extends outside the prostate, the PSA >20, or grade group 4 or 5 (Gleason 8 to 10). There is also a subset of very aggressive tumors is called “very high risk” in which the tumor has extended into the seminal vesicles (T3b) or the rectum or bladder (T4), or there are multiple biopsy samples with high grade cancer.
